Transportation Waiver: I authorize Avenue941 to transport my minor student in a church van or private vehicle, driven by an individual authorized by Avenue941, to and from student activities, between school campus, Avenue941 Campuses and between any Avenue941 Campus and the location of any student activity. I understand my child is expected to follow all applicable laws regarding riding in a motor vehicle and is expected to follow the directions provided by the driver and/or staff or volunteers. I have read, understand, and discussed with my child the following rules: (1) My child will travel in a motor vehicle driven by an adult and my child is to wear their safety belt during travel (if vehicle so equipped); (2) My child is expected to listen to the supervising staff/driver, to respect staff and other children, the vehicles they ride in, and the people they travel with during the trip; and (3) my child is to remain in their seat and not be disruptive to the driver of the vehicle. I have read this waiver and fully understand it, and agree to be legally bound by its terms.

Walker Waiver: I authorize Avenue941 to walk my minor student by an individual authorized by Avenue941, to student activities, between school campus, Avenue941 Campuses and between any Avenue941 Campus and the location of any student activity. I understand my child is expected to follow all applicable laws regarding walking on sidewalks and crossing any streets and is expected to follow the directions provided by the staff or volunteers. I have read, understand, and discussed with my child the following rules: (1) My child will walk safely, adhering to any crosswalk rules or crossing guard guard instructions; (2) My child is expected to listen to the supervising staff/volunteer, to respect staff and other children, and the people they walk with during the trip; and (3) my child is to enter the church or ministry building safely once they have arrived. I have read this waiver and fully understand it, and agree to be legally bound by its terms.
